Could it be ear mites? I know when cats get ear mites, their ears smell and it's very itchy. Also, they ususally have a lot of brown ear wax in their ears, sometimes even flakes outside their ears.

When you put the drops in, be sure to massage the ear so that the drops distribute down into the ear canal.

Not necessarily ear infections can be very difficult to treat and if at first drops alone don't work (they usually rx Surolan) then you need oral antibiotics as well.
